Last Will & Testament
You should determine and provide the instructions for passing along your estate and property. A properly-prepared will states your intentions and will be enforced by the court exactly per your instructions.
Trust Agreements
Preparation of living trusts, land trusts, and anonymity trustsĀ as well as placement of deeds of real property into trust and pour-over wills.
Power of Attorney
Should you become disabled or are unavailable, a power of attorney enables someone to represent you and/or your estate. You may designate powers of attorney to whomever you wish, but people typically choose their spouse or other family member.
Durable Power of Attorney for Health Care / Disclosure to Durable Power of Attorney for Health Care
Should you become incapable of addressing medical decisions for yourself, a Durable Power of Attorney takes effect while you are still competent and continues in effect when you become incompetent. The Disclosure to Durable Power of Attorney for Health Care allows the person you designate as your agent to make specific or all health care decisions for you in accordance with your wishes, including religious and moral beliefs, when you are no longer capable of making them yourself.
Directive to Physicians
A Directive to Physicians allows you to state your desires about medical treatment should you become incompetent to make such decisions. It includes specific and binding directives to your doctors concerning treatments that you wish or do not wish to receive.
